Assertive Community Treatment (ACT)
Consists of a community-based group of medical, behavioral health, and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness.

Community Support Team (CST)
A Medicaid service that offers adults with mental illness and/or substance use disorders a structured team approach to increase and restore the individual’s ability to live successfully in the community.

ASSERTIVE COMMUNITY TREATMENT TEAM
The ACT Team is comprised of a Psychiatrist, Team Leader, Registered Nurse, Licensed Substance Abuse Specialist, Peer-Support Specialist, Vocational
Specialist, Qualified Professional, and Paraprofessional. Together the team creates a person-centered plan for each individual comprised of goals for each
client to meet. ACT is provided 24hrs/365 with a dedicated crisis line and team members available throughout. With ACT you get support in areas like medication management and education, vocational training, and individual and group supportive counseling. As an ACT Team client, you will have clinicians and staff members who will work with you one-on-one to reach your goals. -
ASSISTED FAMILY LIVING (AFL)
Alternative family living allows persons with Intellectual Disabilities to live in a residential setting with professional supervision. The supervisor known as the AFL provider trains in many areas to ensure the client receives quality care.
